NGAL is Neutrophil gelatinase-associated lipocalin for short, NGAL is one of the lipid carrier proteins. Recent studies have discovered that NGAL is an early indication of both diabetic nephropathy and acute renal damage. NGAL performs a wide range of tasks, including binding and transporting hydrophobic small molecules, taking part in signal transduction, inflammation, embryonic development, immunological response, and the emergence and growth of different cancers. Blood and urine tests can be used to identify NGAL.

NGAL is a single molecule used to diagnose AKI, unlike the diagnostic technique of grading standard. According to various researchers, among many indicators, NGAL is the best predictor of AKI. The average level of NGAL in healthy persons' urine was 5.3ng/mL (range: 0.7-9.6ng/mL), while the average level in their plasma was 63ng/mL (range: 3-106ng/mL). NGAL levels rose with renal damage (within 2h). The mean plasma concentration of NGAL ranged from 25ng/mL to 3491ng/mL, and the concentration of NGAL in the urine of randomly chosen ICU patients ranged from 110ng/mL to 40000ng/mL. When plasma or urine NGAL levels are greater than 400ng/mL, acute renal failure is determined to be present.
